Description/Details

The Community Health Team is a service of Nova Scotia Health in partnership with the IWK offering wellness programs and services in the greater Halifax area. Navigators are health care professionals who know health care, community and government systems. Navigators work with adults, children, youth and families to find the right resource for you. This service is available to those living in the greater Halifax area. Call or email for more information on one of their four locations.

What they do

You can join free group sessions or get one-on-one help from healthcare professionals to support your mental health, physical well-being, and everyday routines.

You can book directly by phone or email. You do not need a doctor’s referral.
